Position Description

I. Position Summary: 

In collaboration with the attending physician, the Geriatric Nurse Practitioner Care Manager will provide care to patients at one or more clinical venues of Hebrew SeniorLife. Services will focus on the management for individuals with cognitive concerns and dementia; along with their caregivers.

 II. Core Competencies:

  • Expertise in the medical and psychosocial aspects of memory disorders, and the ability to integrate the clinical, social and behavioral complexities of dementia diagnoses.

III. Position Responsibilities:

  • Performs initial and periodic cognitive assessments of patients and appropriately identifies and documents problems and issues in the medical record.
  • Refers and coordinates patients’ care with other health care professionals and community based organizations as needed.
  • Provides dementia care management to meet the patients’ health needs and coordinates care and services to support the needs of patients’ caregivers.
  • Creates, implements, and oversees multidisciplinary plan of care in a safe and effective manner.
  •  Monitors for disease progression and complications.
  • Evaluates effectiveness of previously implemented treatment.
  • Leads multi-disciplinary clinical rounds and ensures documentation of team recommendations in such a manner that can be billable and enhances patient care
  • Documents patient visits within a timely manner (typically within same day) using the appropriate medical record system
  • Identifies utilization and service delivery problems and participates in their resolution.
  • Participates in quality assessment activities including but not limited to record reviews, outcome measurement and review and revision of policies and procedures. Provides education and training to patients/residents, caregivers and staff in relevant topics.
  • Serves on appropriate committees; participates in staff meetings.
  • Develops a line of care or clinical service in lifestyle modification and coaching.
  • Develops and implements a holistic assessment of overall health/cognition.
  • Assists with implementation for a neuromodulation clinical program.
  • Oversees the development of educational materials for patients and families, including brochures, booklets, website, blogs
  • Assists on development of a Brain Heath course for professionals
  • Defines and oversees integration with other programs at HSL – including for example Great days for Seniors; Vitalize 360; specific memory support and expressive therapy programs across HSL housing sites, etc.
  • Enhances professional growth and self- development through participation in research activities, other professional activities, educational programs, current literature, in-service programs and workshops.
  •  Collaborates with the program medical director and administrative director regarding Center program development.
  • Collaborates with colleagues within center and across the HSL system to access care for patients
  • Participates in strategic planning for program development offering opportunities for growth
  • Participates in the development of technology to support cognitive assessments.
  • Leads Program for Lifelong Health and Wellbeing, an innovative initiative to enable early detection of cognition and memory problems through annual screenings and integrating with other programs at HSL
  • Participates in grant funded activity currently in session and seeks additional grant opportunities with focus on patient brain health, cognitive management and caregiver needs.
  • Engages in definition of new projects 

The above covers the most significant responsibilities of the position.  It does not, however, exclude other duties, which would be in conformity with the level of the position. Completes special projects as assigned.

IV Qualifications:

Master’s Degree required. Graduate of an accredited Nurse Practitioner program.

Licensed as nurse practitioner by the Commonwealth of Massachusetts Board of Registration in Nursing.

Certification: National Certification as Geriatric, Adult or Family Nurse Practitioner. 

·  Four years of Nurse Practitioner experience preferred.
·  Two years as nurse or nurse practitioner in geriatric facility preferred.
·  Experience working with people living with dementia and their families preferred.
