Position Description

Carleton-Willard Village is a not-for-profit, Continuing Care Retirement Community (CCRC) set amid 72 acres of pristine fields and forest. Our campus includes Independent Cluster Homes and Apartments, Assistance-in-Living, Skilled Nursing Units and an Out-Patient Clinic. Position Summary: The part-time Nurse Practitioner offers medical/clinical services to the Residents of the Nursing Center three days per week.

Responsibilities:

  • Functions as a nurse practitioner within the scope of practice established by guidelines and protocols with the Medical Director, the Primary Care Physician, the Clinic Nurse Practitioner, the Director of Nursing and Carleton-Willard Village (CWV) policies and procedures.
  • Provides medical/clinical services from 8 am to 4:30 pm. Monday, Wednesday and Friday in the Nursing Center including regulatory visits as required and urgent care of medical needs as they arise. Review all labs, diagnostic studies and consultant recommendations and provide necessary orders and follow-up.
  • Participates in interdisciplinary team and family meetings for resolution of complex care problems when indicated. Participates in discharge planning meetings as needed.
  • Provides individual and family teaching and counseling as needed.
  • Enters orders electronically through Sigmacare.
  • Consults with Primary Care Physicians as indicated on the individual care needs of their residents.
  • Consults with Medical Director/Supervising Physician on specific cases, or other matters, as needed.
  • Participates in problem solving of medical/clinical issues with nursing and other clinical staff.
  • Supports the mission, philosophy and goals of Carleton-Willard Village.
  • Performs other related duties, as required.
  • May provide services to the Residents of Llewsac Lodge, Apartments and Cluster Homes when covering for the Clinic Nurse Practitioner.

Qualifications:

  • Nurse Practitioner licensed in Advanced Practice by the MA Board of Registration in Nursing and credentialed by the American Academy of Nurse Practitioners (AANP) or the American Nurses Credentialing Center (ANCC).
  • Master’s degree in Nursing with education and training in Geriatrics.
  • Five years of Nurse Practitioner experience, including two years in a geriatric setting.
  • Excellent communication and organizational skills required.

This is a part-time, 24 hour/week position with potential for Saturday and extra per diem hours.
